UNDO PATCH of VMS Update #12 returns error

I did an UNDO PATCH of VMS831H1I_UPDATE-V1200 and towards the end this was displayed:

%DELETE-W-SEARCHFAIL, error searching for PCSI$DESTINATION:[SYS$LDR]DO_NOT_DELET

E_UPDATE-V1200_SYS$EFI.BAK;

-RMS-F-DEV, error in device name or inappropriate device type for operation

%PCSI-I-SPAWNEXE, error executing: @pcsi$source:[sysupd]POST_UNDO.COM

 

%PCSI-E-EXEPOUFAIL, product supplied EXECUTE POST_UNDO procedure failed

-DELETE-W-SEARCHFAIL, error searching for !AS

%PCSIUI-E-ABORT, operation terminated due to an unrecoverable error condition

 

I followed this with the UNDO PATCH of VMS83IH1IPCSI V3.0 without any problem.

 

We did a reboot of the servers (cluster) and the PRODUCT SHO HISTORY does not show the VMS and PCSI patches anymore.

 

What actions can we take, if any, about the message (EXEPOUFAIL)?

 

DO we have to perform some tasks before doing the UNDO PATCH? What is the proper way to do the UNDO PATCH for the O/S?

A few days prior to execution of UNDO PATCH, I did a DELETE RECOVERY of Patch V10 and PCSI V2.0.

 

After the installing patch V12, this is the product history:
