From the information provided:

Sense Key 4 -> Hardware Error
ASC/ASCQ=409D -> Write Calibration Error
Internal Error Code 0789 xxxx -> Laser Power Calibration Failed

Possible causes are:

1) The drive may have a slowly dying laser, or a dirty objective lens (which cannot be cleaned).

2) The particular media may be dirty.

3) The library environment has contamination.

4) A combination of the above.
