Bootable CD-ROM

I've never posted in these forums before, so I'm just guessing that this might be an appropriate forum for my question.

http://h20000.www2.hp.com/bc/docs/support/UCR/SupportManual/TPM_ecg0720298/TPM_ecg0720298.pdf has an explicit reference to "White Paper Compaq SmartStart, Doc. No. 703a/0297".

I've not been able to locate that paper using the Search engine.

I'm interested because TPM_ecg0720298.pdf states that Compaq had bootable CD-ROM support back in 1993 prior to El Torito, so I would like to know:

1. When was that support first implemented.
2. How did the CD differ from what was later El Torito.
3. I ASSuME that all systems now use El Torito.
