SIM 4.1 Disk failure notifications

I was running SIM 4.0. I was told that hdd failures not being reported in SIM was fixed in 4.1. So I started fresh and installed SIM 4.1. This Monday we had a disk drive failure...no notification from SIM. Didn't even pick it up. Anyone know what is going on? Thanks,
