BL25p with "error on PCI configuration" on the HBA

I have installed quite a few BL25p with Q-logic fibre cards. But recently i've had a few which on the POST. reports the "error on PCI configuration" and it wont let me CTRL-Q to look at it. I've swapped the card with a working fibre card, and it passes the POST. Anyone know if this is a known fault, or if there is a way around it?

wrong firmware used for QLogic ?
What is installed NC370i or NC77xx series ?
System ROM ?
reseat NIC ?
a failed Card has same problem in 2nd server ?
