Event notifications not working correctly

I am having trouble with the event notification system. Are there restrictions on how many systems can be watched or on how many tasks an event can trigger?

I have 3 groups: one that receives major/critical for all servers, and two that receive major/critical for their select servers.

Insight manager is triggering the all server group when something goes down, but not the smaller groups, even though the server is in both lists and both tasks are properly set up.

Any ideas on how to fix this?

To make things even more confusing, it triggered on a different server going down. So far this task is 1 for 3 in accuracy.
