Strange H/W paths & alt. path failure

Environment:
(2) HP 5470s running 11i - both identical in OS and patch levels (at least from what I can tell)
(1) 2C2D EVA5000

Added 2 LUNs and presented one to 'server A' and the other to 'server B'. A quick ioscan shows the new devices, but on server A, there appears to be 4 new drives, while server B only sees 1 device. Furthermore, the H/W path is a bit strange (see attachment). Upon investigating using the spmgr utility (spmgr display), server A complains about an 'incompatible command'. It works fine on server B (this output is also included in attachment).

When attempting to create a VG on either server, both complain about not being able to build an alternate path. Additionally, the VG never gets created on server A, but works fine on server B.

Does anyone have any ideas as to what is causing this behavior? Is it a patch level? Perhaps a corrupted installation of the HPUX Kit or HPUX Secure Path?

in the host 'A' output I guess that secure path did not attach HSV volumes to it and did not translate 4 redundant LUN paths into one as host 'B' did. The first thing I would check if host 'A' has 'Hewlett-Packard HP-UX' OS property set. Log into SAN appliance, go to hosts and check host 'A' properties. In addition you can compare host 'A' and 'B' properties to see if there're any differences

I checked the host configuration for both servers in Command View and validated that they are set to HP-UX. The EVA is something new to me so I'm not sure what ioscan is going to report. Does it use the 255/255/0.0.0 H/W path or the path pointing to the actual HBA slots (e.g. 0/10/0/0.1.0.0.0.0.1)?
