Solution
I'm pretty sure that the cable specs are the same as for the old D series EISA MUX card (J2482A).
Document id A5000807 in the knowledge base describes the pinouts, do a boolean search for 'MUX and RJ45 and pinout'.
I don't think that the information will paste too well so I have attached it as a text file.
