the switch is not sending the power to the PoE AP (MSM460).
Log info:
W 11/07/16 15:50:14 00732 chassis: PoE controller 4 selftest failure.
W 11/07/16 15:50:14 00732 chassis: PoE controller 6 selftest failure.
W 11/07/16 15:50:14 00732 chassis: PoE controller 1 selftest failure.
W 11/07/16 15:50:14 00732 chassis: PoE controller 5 selftest failure.
Also lights are flashing amber on these ports.

Log a call for that. It has a lifetime warranty!
You may also want to try a power-cycle. Depending on the actual nature of the problem, it may clear one or more of them. Try the POE device on a different port too; maybe the device(s) have a problem. I have seen APs that have run in over-temp conditions do that occasionally.
