disks gone missing from eva

after i presented disks form hp eva san to my sun server after 2 or 3 weeks then went misssing after i done a reboot then went online
i would like to know what happened this is some of the logs and out put

any ideas please thank you i have solved it on one node
and left it on the other node to see why and what happened
