Re: storage connection failed with mpio failover software
You missed the attachment...
Are you receiving errors on all servers or only on specific server or only on Windows hosts?
This will lead you whether to check the server or the infrastructure. If you are receiving it on all the servers, use HPDSM DEVICES command to check which path is failing. And if it is significantly path-A or path-B on all the server, check whether it's a problem on SAN Switch (you didn't mention but I assume you have redundant SAN fabrics). Or may be on the EVA, check Controller termination log whether any of the HSV controller rebooting or down.
My 2 cents worth.

Re: storage connection failed with mpio failover software
It clearly says that the connection for the HBA (Connected in slot 0) has gone down. Check for it's physical connectivity (lights on the HBA). Probably it's a connection issue, which can be resolved by looking into:
- Physical fiber connection to HBA.
- HBA status (if Emulex, check through HBAnywhere utility)
- Check the physical connection at SAN Switch.
- Check port stats on SAN Switch (brocade; run portshow command)
Hope this helps
