The Format Did not Complete Successfully (EVA 4000)

Im Having a issue trying to format a presented lun to one of our windows 2003 nodes x86 ..
I have no issue getting the node to pick up the LUN nor to initialize the lun ... i start the Partition wizard and everything seems to go well until the format process .It seems to hange for a while then finally comes back with the Listed error "The Format Did not Complete Successfully " and other times i have to un presented the lun in order to continue normally server functionality .I have updated all drivers (HAB 2640 DULE Qlogic ) from hp web site , F/W and all hp apps ..mpio, and so on, I have also tried disabling windows auto mount ...Possible bad HAB ? Any help would be appreciated.
I also Recreated the Node on the San side ,zones and all .

EMAN,

Have you checked the HOST settings at EVA? You should have selected OS TYPE - "WINDOWS".

If you are suspecting HBA then either you should look under SANSurfer utility or SAN Switch port, check if there are any port Tx/Rx errors?

Other than that it's purely a Windows issue and you should provide more details, what parameters given for formatting etc.

I did check all the Settigs in command view for this Node .Eeverything is set correctly ..the OS type is set to windows .

As for the Format Parameters (100gb lun )i took defult settings ....Quick format NTFS ,Defult Allocation size.

Im starting to think the same .."Windows Issue"
