rx4640 running 11.23
The system is not booting. The boot menu is empty, it's saying:
Boot Menu
and doesn't give me any choices to boot. What could have caused this and what can I do to recreate it?

run the EFI shell, locate your fs(x) device.
fs0:> cd efi/hpux
fs0:\EFI\HPUX> hpux

Press Any Key to interrupt Autoboot
...
(don't press any key)
Once the OS is running, use "setboot" to re-create the boot menu (this is one way to do this).

May be the boot Entry from EFI boot Menu is deleted.
You can try to manually add the entries in
Boot option maintenance menu:
-> Add boot options

Everything was fine until the machine was brought down for maintenance for 20 mins and after that, it failed to boot, it "forgot" the boot info.
