clustering lt6000,4 RS/12, netraid 4m, windows 2000

Installing a cluster of 2 lt6000 servers, 4 RS/12 & netraid 4m controllers. We followed all details mentioned in HP netserver installation guide of netraid 4m & microsoft windows 2000 (clt_MSCS4Mig.pdf) issued on november 2001. after getting done with creating shared storage resources, when failing over & failing back, some resources fail. The current status is that node 2 fails 2 see one of the hard disks in the shared storage. The installation and FW & SW is from navigator CD M.04.05. When running diagtools both servers fail PCDI test returning: PCI Bus transfer using Bus Master Cycles failed
Error Code : 008A
when node one is booted, when booting node 2 it sometimes returns fatal error: controller monitor failed. array controller not started. when restarting it takes a whole lot of time and sometimes it passes. one other thing is that one of the containers consists of 8 73 GB mirror 10. it fails consistency check on the low level container but passes it on the high level container. all the diskset resources are made in the cluster group.

I now have both servers give this message
Fatal error: controller monitor failed
Array controller not started.

Just before this message one of the RS/12s blinked hysterically. would this cause both array contollers to fail & need replacement?

The fatal error message stopped when I turned of one of the RS/12.

I now want to know why one controller cannot see a hard disk placed in ID 0.10 while the other does. I tried replacing the hard disk.
