What is the ioscan output showing. If the disk is sensed by the server and is seen through ioscan, you should be able to add those disks in the vg without any problem.
ioscan -fnC disk
or ioscan -fnH Hardware_path

Also check whehter the disk you ahve pulled is not a gate keeper , may be you can use the inq or syminq command to check the same .

The sequential numbering is certainly with LUNs ate frame and is resolved with new patches. I have my frame with non-sequential luns working with patches.
Use EMC inq utility to make sure you are able to see all disks.
Couple of times I had trouble getting new EMC luns recognised by HP and multiple #insf -e, command helped me overcome the problem.
