Please note that this is a G50 class and the root disk sitting on the same bus as the dat and cdrom.
Cannot find ENTRY_TEST.
Failed to initialize.
ENTRY_INIT
Status = -4
Failed to initialize.
I have checked for cable connectoins, and tried another disk ,with no luck.
any help welcome,

Solution- Problems related to the scsi termination, the scsi cable itself or scsi connection.
OR
- If you're booting from disk, there may be some hardware problem with the disk.
If you have an external DAT you may want to give that a try.

I have checked for cable conn's and also replaced the disk. I even tried comming up from boot alt with ignite and no luck

a) check for bent pins on the connectors
b) replace the terminator (they do go bad)

It needs to be the same as the original.
Most original drives are set at 6, most replacement drives are set at 5 as they are most likely used as an additional drive not a replacement.

Did you check the SCSI fuse? If it's ok the termination LED on the multifunction I/O card will be on. If not replace top-hat fuse(2 Amp)on the card.
Disconect the disk and see if you can get to the ODE on the support CD. Run MAPPER.
The disk you installed may already be terminated in which case remove the resistor packs on the disk.
