boot problem,swap error

Hi folks,
after I boot my server, following message has appeared and system halted

swap device table (start&size given in
512 byte blocks)
entry 0 major nmber is 64,minor is 0x2;LVM:vg[0]:pvnum=0 (dev_t=0x1c015000) is POWERFAILED

what is wrong ??

the disk at teh address supplied in vg0 (root vg) is turned off or has failed or is unreachable.
Check your disc at that hardware address related to the dev_t entry

I agree with Melvin ... H/W is the most likely cause.

However we don't no much about your setup ...

is this a MC/SG cluster or just one server ?

any additional clues in syslog.log ?

Do any other luns/devices report POWERFAILS at any othertime?

What is the pvtimeout set to for each of the PV's?
The default is 30secs which in the case of arrays and a heavy I/O load, could be too short.
Values such as 60,90 or even 120secs are not uncommon.... may be worth a try before going down the h/w route ?

Verify SCSI termination.

thanks for ideas,
problem found (of course,somewhere else)

RAID 7 has been connected to the system in wrong way (terminator and scsi cable)
