i have been unsuccessful accessing the codeword request form, online or from codeword@mayfield.hp.com. any pointers?

I've had success going through http://licensing.hp.com/index.slm.
If that doesn't work, there's always the telephone.

http://licensing.hp.com is the place where you can get codeword for the product if it is listed in SLM
