Usually for any LV that wasn't created with the contiguous flag (e.g. lvol[1-3] of vg00) LVM simply takes the next free PEs available of a contained PV during an lvextend.
So there is nothing special about concatenation.
If you run out of free PEs just get a new disk, pvcreate a PV and vgextend it into the VG of whose LV you need to lvextend.

The biggest difference is that the striping is by LV extent, i.e. 1MB or 2MB, not 2k, 4k etc..
Read the man page about lvcreate -D and -s options.
Concatenation is the joining two or more sets contiguously or in array speak two or more disks are joined together back to back, filling from the beginning of disk 1 then continuing on to disk2 once disk 1 is full.
