Are you able to tar / cpio / fbackup to the tape.

It appears to be having problems with your tape device. Are you able to write to it from another application? You could try to tar something to it just to prove that it works.

Try running a cleaning tape through the drive and then run make_tape_recovery once again. I've seen this being the issue for failed recovery tape creation numerous times.

SolutionYou may have a problem with the drive.
If its new, that drive is self terminating, don't put a terminator on it.
Before declaring it dead, go through the prior posts and make sure those insf drivers are solid.
Also, check the cabling and that its connected to a LVD scsi card.
I'm not thrilled with your Ignite syntax, but any of the suggestions in prior posts are worth re-testing.
