When I perform the following command:
dd if=/dev/rdsk/c0t6d0 of=/dev/null bs=1024
I get the following output:
dd read error: I/O error
6930+0 records in
6930+0 records out
What could be the problem and how would I resolve this.

Looks like a hardware problem. Replacing the disk would be the best option.

Check the disk is claimed in an ioscan.
Check syslog.log for any error messages.
It does look as though you have a faulty disk.

Please find attached the ioscan (no real problems in that) i ahve also ran cstm on the devices, doesn't show any problems!
All the lvols on that disk are fine.....??? and u guys are right may need disk replacing!
let meknow if there are ant other tests that can be carried out!

Try using mediainit on your disk to low level format it in an attempt to fix the bad block;
mediainit -v /dev/rdsk/c0t6d0
This is a DESTRUCTIVE disk so the disk cant be in use at the time - remove it from any volume group. If it succeeds then your disk is aok - pvcreate -f it and add it back into a VG and reuse. If mediainit fails your disk is dead and must be replaced.

We have had problems in the past, where the disk is calimed in an ioscan, but unavailable in LVM.
In this instance we replace the disk & re-sync the mirrors.
