/dev/dsk/disk_query ???

Using sam I was creating lvs and I got errors about a device
call /dev/dsk/disk_query

Why is this device present?
What created this device?
This is a serious bug

Sam uses this file temporarily but will remove it after it has finished. If you are getting errors to do with this apply PHCO_21187
Minimum effort maximum output!

It's supposed to be a temporary file used by SAM.

You can just delete it and try again.

The latest SAM cumulative patch ensures that SAM removes it when it has finished with it.

I think the disk_query device is a temporary one created by SAM to talk to a disk before allocating/using it. If you exit from SAM and this device still exists then you should remove it. If you do a full ioscan -fCdisk does it list any disks as being NO_HW (not there anymore) or another status than Claimed ? If so then you have a problem with a device which is causing the SAM error.
Some reports this is a SAM bug, do you have the latest SAM patch installed ?

It's a bug, because it caused me to lose lv data and that
prevented me from mounting and using those lvs.
