First question, does that disk actually exist and is it supposed to exist.
If its just a ghost the following plan might help.
mv /etc/lvmtab /etc/lvmtab.old
vgscan -a
Better yet, since I'm suddenly thinking answer one won't help(or hurt), I'm thinking...
rmsf the device file of the ghostly disk from /dev/dsk
See if it comes back after boot.
If it does, talk to your SAN/Disk array administrator.

Recommendations: full backup, installation of latest FC driver, Diags, GR and HWE patch bundles from CDs of same release, but not earlier than Sep 2002
