Disk I/O - what file is causing it
In Glance, when drilling down to I/O by lvol, I see that the /opt mount point has a lot of lv writes compared to other servers here. Is there a way to determine which file or process on that mount point is doing the most writing to the disk? I want to determine if it is HP-UX or an application causing the writes.

You should be able to get a handle on the process by drilling into glance.
As to what resources the process is using, I would suggest lsof.

You will have to set suid on the binary to get the install to work right.

Re: Disk I/O - what file is causing it
You must do a
fuser /opt
as root
then you will be shown a few process id's.
Do a ps -ef | grep "number" (number is the process id you will get through fuser command)
Then you can look it up in another window through top and see if the process id is using a lot of CPU.
