SolutionLooks like you have a faulty disk at 10/0.5.0 this should be the internal disk in the server cabinet. Do a complete ioscan (ioscan -fn) and see if something shows up at 10/0.5.0 . Also try to do a diskinfo for /dev/rdsk/c4t5d0 and see if something shows up
diskinfo -v /dev/rdsk/c4t5d0

We have 2 dead drives this year.
And exactly after end of 3 year hardware support.
And, BTW, these disks have some problem - it is not possible to save crash-dump on these drives. This is a known problem.
