diskinfo result strange...
Is it normal to have block per disk to be 0 under the command "diskinfo"
e.g.
"SCSI describe of /dev/rdsk/c0t5d0:
vendor: IBM
product id: DDRS-39130WS
type: direct access
size: 0 Kbytes
bytes per sector: 0
rev level: HP01
blocks per disk: 0
ISO version: 0
ECMA version: 0
ANSI version: 2
removable media: no
response format: 2 "
could this be that the disk is not mounted? anyway I did encounter problem booting up with a LV inside this PV.

Where is the disk located?.
As you can see it's size is 0. Looks like you have a bad disk. On XP/Hitachi storages you will see LUN 0 devices with 0 bytes called placeholder LUNs and the description of it would say "DISK SUBSYSTEM".
Yours doesn't seem to be the case. So, I suspect you have a bad disk in slot 5 of your disk (possibly Jamaica??) array.

I've taken to getting our SAN admin to setting up an actual lun and presenting it as LUN 0. this makes that problem go away.

1.what is the o/p of #pvdsiplay -v /dev/rdsk/c0t5d0.
2.What is the o/p of ioscan -fnC disk : is it showing claimed HW
