Could you check your tape, clean the tape drive and even try a new tape.

SolutionAs suggested either clean the tape, mediainit or try a new one.
I also would add that you upgrade your Ignite to reasonably latest version. make_recovery has been outdated and if you have newer systems, there is a good chance that the recovery may not work.

If cleaning the tape drive again doesn't solve the problem, then either try a new tape or run mediainit on this tape & try again.

Do I need Quality pack march 2004 ?
Is possible verify that the DAT drivers are OK ?
On the "ioscan -fnC tape" command it seem OK.

I try again make_recovery but I have the same error.
In the ignite file log I have the follow error:

but if I run the stm utility and I verify tape, it's OK!
in the root mail I have the error msg as the attachment txt file.
I don't understand the mistake because if I read about the event#559 on the web site http://docs.hp.com/hpux/onlinedocs/diag/ems/dm_stape.htm
seem it is maybe no a hardware failur.
It is possible to attempt drivers reconfiguration ? How can I perform this operation ?

While attempting to process an I/O request, the device reported a mechanical positioning error.
Probable Cause / Recommended Action:
The device has experienced a hardware failure. Repair or replace the device, as necessary.
Think it is best to log a call with your locale HP response center.
