#mv /etc/lvmtab /etc/lvmtab.org

move the /etc/lvmtab and run again.
#mv /etc/lvmtab /etc/lvmtab.org
then
#vgscan -a

Is there a disk like that and present?
Also, you should not be running vgscan -a directly. run vgscan -pva first, then if required run vgscan -a. Also backup old file first.

Is there some mismatch for device files under /dev/dsk and /dev/dsk ?

after there is only vg00 and vg01 / vg02 is missig
