Didn't understand the last part of your question, but upgrade your make_recovery to new make_tape_recovery. You can download it from the HP site under Ignite-UX section. make_recovery is getting old and the new version has lot more options and more reliable.

eg: make_tape_recovery -I -v -x inc_entire=vg00 -a /dev/rmt/0mn

This command will create the recvery tape on /dev/rmt/0mn of root volume group (vg00)

It's very difficult to uderstand the end of your question, but in any case:
1. Rewind the tape: mt rew
2. Clean up the tape drive with a special cleaning tape
3. Try to use another tape.
