Error when installing HP-UX 11.23

I was installing HP-UX 11.23 in RX7640, when installation at disc 1 still fine. but after insert disc 2 i got error warning at step :

*Installing fileset "Ignite-UX.FILE-SRV-11-00,r=C.7.2.94" (102
of 194).

DIAGNOSTIC SYSTEM WARNING:
The diagnostic logging facility has started receiving excessive
errors from the I/O subsystem. I/O error entries will be lost
until the cause of the excessive I/O logging is corrected.
If the diaglogd daemon is not active, use the Daemon Startup command
in stm to start it.
If the diaglogd daemon is active, use the logtool utility in stm
to determine which I/O subsystem is logging excessive errors.

how to resolve it? do i have to reinstall again from disc 1?

Sounds likes either the second CD is bad and you are seeing read errors OR you just hit a bad spot on the target disk and you are getting write errors.

Can you try to read the second CD on a different system?

yeah i think disc two is bad... and i started from the begining to install.
