Have you presented the Virtual Disk to the host?
Did you check your zoning configuration in the switches (if used)?

I have presented the lun to the host from command view, configured the HBA and installed a driver for A6295A.

If you can connect to your switch, use:
switchshow
And ensure that all ports are online and you can see the WWID of the HBAs.
cfgshow
Check your zoning configuration
