Re: Fail to reboot
Take a look at the thread below, This link refers to the K-Class troubleshooting guide. The error codes for k-class are there at the bottom. Maybe they will help you isolate the problem on your D-class too,
http://us-support.external.hp.com/cki/bin/doc.pl/sid=2abe97c116c2a8e82e/screen=ckiDisplayDocument?docId=500000000083759
Hope this helps.

Re: Fail to reboot
Restoring an ignite tape should work, but you may run into troubles if it was cut from a different server.
Generally speaking you should be able to recover from any ignite tape as long as it was cut on on equal type of server. 'D' class and 'K' class should be okay together.
You should always have a created ignite tape as a backup for each and every server where possible.

Re: Fail to reboot
I would suggest you log Hardware case to HP Solution Center.
They need to decode FLT code.

Re: Fail to reboot
I was getting a FLT 3000 originally, which happens earlier in the boot cycle.
My problem is that we don't have HP support on this box. So I have to go it alone.
