I was running a large restore overnight from a DLT on /dev/rmt/0m but it failed part way through. If I now try to pull the index off the tape I get an error;
frecover(5403): Unable to open or identify device at /dev/rmt/0m
And if I try to rewind the tape I get;
/dev/rmt/0m: I/O error
What could be the problem ?

Solution
Your tape or drive encountered an error whilst doing your frecover. Probably due to a dirty tape or drive or a bad tape. Probably you only need to clean your drive with a cleaning DLT and then reload your tape and retry the recover. Only if you keep getting the same error (frecover keeps failing at some point) is the media bad and you should replace it.

Hi,
Try cleaning your tape drive first. If the problem persists at the same place on the tape the tape is probably no good. Also check the contents of some other tapes. If the problem persists with the drive get it replaced.
