Getting I/O device error and POWERFAIL error

Hi,

I am installing 11.11 OS on N Class machine using external DVD ROM, but I am getting following errors. Pls can anybody help me. I checked the SCSI termination, it seems to be fine.

Hi Ravindra,
Look like some I/O device is in problem. You need to check the SCSI Adapter, Root Disk and SCSI Cable connections/terminations.

You can go into GSP and check for error.
From console, press B to go to GSP.
GSP> sl
select E
View all error. This may direct you to the root cause of the problem.

Hi,

It seems to me there is a problem with disk c1t15d4, if such a thing exists on your machine.
In order to clear out the situation, could you please disconnect from the computer all external devices, check scsi connectors and terminators and try to install OS once more.

Some servers of this era included a drive cage through which scsi bandwidth and power was routed.

This parts failure can cause drives to appear to fail when this is in fact not the problem.

A thorough identification of every device on an ioscan -fnC disk is needed to make sure all are present and accounted for and no scsi id conflicts exist.

Hi,
At the BCH, do a scan or discovery of all the H/W path. Then check it with what should be there.

Also mentioned above, check the chasis log in the GSP/VFP.

Hi Ravindra,

From the log you sent here, the error=5 is an I/O error on the disk device you are using for installation.

I would first suspect the disk. If you have another disk, try installing the OS on the other disk. I presume you are using the internal harddisk for OS installation. In that case SCSI cable related issues are rare.

If you have an external disk then check the SCSI cabling too.
