It is possible that your tape is bad. Try (if possible):
# mt -f /dev/rmt/0mn fsf 1
# tar -tvf /dev/rmt/0m

What server is it?
What OS version is running?
Do you backup and restore to the *same* machine?

What type of machine are you restoring to?
The 'ISL booting hpux (;0):INSTALL' indicates a 32-bit install if I recall correctly. If you are backing up a 64-bit and trying to restore to a 32-bit, then that dog won't hunt.
