the reasons that u stated must surely be the cause of such an error. However since this error is not causing much concern to the people onsite i won't be able to ask them if it is 2) or 3)
i.e. if they forgot to fcmsutil /dev/td2 replace_dsk 0x20200
OR
the switch port has been changed

i tried ioscan without any arguments but the status of the drive remains same "NO-HW"
anyways your suggestions helped me a lot in understanding the above concepts. Thank you.
