HPSD 4 - How to automatically link Form and Template ?

I create a new CTT form (eg Planned Outage) to show only the fields required and a template to apply defaults to the fields.
Can it be set up so that when the new form is entered a particular template is loaded ?
It appears that one template is assigned the default and is the default for all CTT forms.
