The ioscan shows this to be a "decru" device.

has anyone seen this issue?

dmesg won't show u the timestamp so we really don't know when this happened.

Check syslog.log for similar message like this and run ioscan to check if all the devices are CLAIMED.

check if there are any lbolt error messages in syslog

all drives are claimed. There are no other messages in syslog (no lbolt, etc)

My guess is you seem to be using:

DataFort SAN/NAS FC Series Storage Security
Appliance by Decru
(possibly part numbers FC520(2U) or FC525(1U))

What else is on the hardware path 0/2/0?
