I/O hardware probe message
vmunix: Last successful probe at path 0/2/0/0.99.68.255.1.3.0 initiated from 0/2/0/0.99.68.255.1
The ioscan shows this to be a "decru" device.
disk 278 0/2/0/0.99.68.255.1.3.0 sdisk CLAIMED DEVICE Decru
has anyone seen this issue?

This shows up from dmesg output: I/O hardware probe timed out

Check syslog.log for similar message like this and run ioscan to check if all the devices are CLAIMED.
check if there are any lbolt error messages in syslog

My guess is you seem to be using:
DataFort SAN/NAS FC Series Storage Security
Appliance by Decru
(possibly part numbers FC520(2U) or FC525(1U))
What else is on the hardware path 0/2/0?
