I got similar problem, here's the error message in output log:

* Checking Versions of Ignite-UX filesets
ERROR: Failed to open tape device /dev/rmt/0mn:No such device or address
(errno = 6).
ERROR: Check tape device failed.
======= 12/13/06 17:17:08 EAT make_tape_recovery completed unsuccessfully
Since I logged in to the server using it's floating package IP (it's configured HA cluster with another server using MC/Service Guard), I wonder if it will affect backup and I exit, then had another try by directly login to the physical IP of that server.
However, when I relogin to the actual IP of server, and retry the same command, it's OK.
No error reported. Very strange.
I'll have another try next time to find if the problem will occur again.

May filesystem /var full?
think you need at least 250MB te make ux imgage,
use cleanup command:
cleanup -p -c 1
cleanup -c 1
dont forget to cleanup the /var/tmp
