I just installed a dual 100/mb card in a HP K420 HSC I/O expansion slot. Brought system up and configured lan1 in SAM. I could then connect. When I reboot the communication daemon fails and the lan interface configure fails. When I go back into SAM it is extremely slow. When I get to card configure, lan1 is unconfigured. I can configure and connect with no problems. I have done this process several times.

Solution
The configuration data for the J3514A card is located in /etc/rc.config.d/hpbtlanconf.
I'm betting that you have configured for AUTO_ON or the corresponding switch port is improperly configured. AUTO_ON is not reliable; your best bet is to hard set to 100FD on both ends of the connection - i.e. on the host computer and on the ethernet switch. You should be able to make these entries manually using vi.
Next, examine /etc/rc.config.d/netconf and make certain that your IP addresses and subnetys are correct and that you don't have more than 1 interface using the same subnet (unless you are using APA).
The next thing that I would do is look for and install the latest LAN cumulative patches for your OS release.
After this, you should be able to reboot and you should be fixed.

Auto-neg is off on the card and switch. It is fixed at 100fd. I have not edited any files yet. I will do so and reboot.

The system boots properly and I can connect after editing the suggested files, but I still get the following failure:
