Re: ioscan Error have you seen it?, Know a fix?
looking at errno.h:
#define EAGAIN 11 /* No more processes */
So it looks like it had a problem spawning a process or thread.
This appears to be a rare occurrence, and is just a warning.

Re: ioscan Error have you seen it?, Know a fix?
It is indeed a warning
error 11 is eagain.
see http://docs.hp.com/en/TKP-90202/re33.html
It seems that there were too many threads being spawned and it waited until threads exited and then continued.
Rare overload condition but you could change tuning.

Re: ioscan Error have you seen it?, Know a fix?
When you have several applications which require different amounts of open files, you can set maxfiles_lim according to the needs of the "largest" application, and maxfiles according to the needs of a "normal" application.
Before starting the "largest" application, use "ulimit -n" to increase the default "soft" limit (caused by maxfiles) up to the "hard" limit set by maxfiles_lim.
That way, the "largest" application can get the resources it needs and the "normal" applications still get limited early enough if they happen to run amok and start eating resources.
