this can be a failing diks.
can you try reading some data from the disk?
dd if=/dev/dsk/cXtYdZ of=/dev/null
also please see using the pvdisplay /dev/dsk/cxtydz and tell the informations.

Probably the disk is gone bad. Still you can check.
Check the status of the disk through cstm:
# cstm
cstm>map
cstm>sel all <- to see all details
cstm>sel dev 20 <- (20 is the dev num for my disk)
cstm>info
cstm>il
Check the details.
You can also try removing the special files and then recreating it again.
# rmsf -H
# insf -eH
Also try the dd test.

Most probably the issue is disk going bad.
Check from cstm and also perform a full dd test on the disk.

Please verify :
0) instance numbers found in 2nd column of ioscan - duplicate or unique?
a) ll /stand/ioconfig - ll /etc/ioconfig (* permissions, ownership *)
b) ll /etc/inittab (* permissions, ownership *)
c) sdiff /etc/inittab /usr/newconfig/etc/inittab
d) read man page on ioinit, consider running '/sbin/ioinitrc -i' - I believe this will check for differences between kernel I/O mapping and /stand/ioconfig and provide errors found on man page

This is something like a DS 2120, right? So you only have one scsi controller going to both pri and alt boot disks. (* c2 *) But this is a c3 controller. Examine this controller.

You got a tape drive and a disk on one scsi controller - that's a no no - tape drives get dedicated scsi controllers.

Add to your sequence.
insf -C disk
This before insf -e
Makes sure the disk is really there.
Still looks from the original question like the disk is dead. diskinfo is telling me that.

The disk controller is still alive (CLAIMED in ioscan), but the disk mech is already dead (size of zero).
Replace the disk.

Do you know for sure that there is even a disk? Because I read this as there is no disk.
