1 Not upto date on SCSI patches.
2 Loose, improper connections, improper SCSI terminations.
Did you check connections on tape drive? Try doing insf -ev and then ioscan -fnCtape.

Could be a bad card,
Could be termination,
Could be the device is bad.
I call hardware when this happens, thats what they are there for.

My advice is to remove the front cover on the 5300 and press firmly to ensure a good contact between dds and box.
