Disk at patch 0/0/1/1.2.0 (c1t2d0) is possibly bad.

hi

may be the disk driver got removed. try running insf -e command and see if the status of the disk /dev/dsk/c1t2d0 changes to CLAIMED.

Well, the fact that the disk /dev/dsk/c1t2d0 shows a pvstatus of unavailable in the vgdisplay, and shows NO_HW in ioscan are pretty good clues that the disk has died.

If you have a support contract, call HP and get them on site to replace the disk.

If you don't have support, you will need to find an 18GB disk appropriate for your machine (which appears to be an L-series or rp54xx) and replace it yourself.

The "When good disks go bad" referenced above gives pretty good instructions on replacing the disks.

Your disk is gone bad. Call HP to replace the disk . If this is your root disk, use the procedure in "when good disk go bad" to replace it ..

You will need to get the disk replaced and then re-mirror.

Above logs confirms that the disk is faulty and to be replaced immediately..

Hence you are seeing stale messages in vgdisplay. Would recommend you to get the disk replaced ASAP if its a RP54XX series then its an internal disk and the below given is the part number.

Check the /var/adm/syslog/syslog.log file for SCSI-lbolt related errors and paste them here. This would doubly confirm if one of your vg00 disks has gone bad.

We called HP support and replaced the disk. It had failed indeed.
