I'm trying to make a make_recovery tape but I got the error below:
vsurjux1@root:/ #make_recovery -A -C -d /dev/rmt/2m
Option -A specified. Entire Core Volume Group/disk will be backed up.

HP-UX System Recovery
Going to create the tape.
make_recovery(472): /usr/bin/dd failed,res=512,errno=9
Cleanup

Anyone can help me?

It's probably a bad tape drive, or possibly the tape.
see these threads, I used this string to SEARCH with:
make_recovery(472): /usr/bin/dd failed,res=512,errno=9

before your answers I got solutions searching here.
The tape was demaged. but the interesting was it was ok to work with tar or cpio. Only with make_recovery it got error.

Just an addition - The make_recovery has been replaced with the new utility make_tape_recovery. Update your system with the latest patches and you will be able to use this utility. This has more flexibility and options to use with.
