ls -l /dev/rmt/0*
and
lssf /dev/rmt/0*
show ?
Did you check that id 4 is a free scsi address on the bus ?
What is your entire ioscan like ?

Did you check that id 4 is a free scsi address on the bus ?
How to check this.

I would try to attach the tape to this controller:
0/0/2/0 ext_bus SCSI C87x Fast Wide Single-Ended
0/0/2/0.7 target
0/0/2/0.7.0 ctl Initiator
Nothing else is connected to this one according to IOSCAN, which is best for performance and trouble shooting.
Let SAM detect and install the new hardware.
Watch for proper termination of the device

your output:
tape 0 0/0/1/0.4.0 stape NO_HW DEVICE QUANTUM DLT8000
it mean you tape driver is not available. check it again. If you already attached it to system make sure that kernel driver (tape2, tape)is in IN status and issue following command
#insf -e
