ioscan recognized the new disks and controllers with no problems but we have been unable to config the new disks for usage ... tried through SAM and command line
each time we try to config the disk(s), we've been getting a log message of:

the output of the log starts with:

Summary:
Disk at hardware path 0/4/0/0.8.0.255.6.7.0 : I/O request failed.
Description of Error:
The hardware responded to the initial query, but then it stopped
responding to more requests by the driver. The I/O request was not
completed.
Probable Cause / Recommended Action:
The bus device reset may have occurred or the device has failed. The bus
device reset could have occurred because the power was cycled or that the
device is a part of an enclosure and a device in that enclosure was pulled
out or put in, or that the interface card had a problem and it reset the
someone suggested that we need to low-level format using Command View but I'm almost certain that we didn't do that with the 1st 6 disks (in fact, we don't even have command view)
any thoughts?

Where did you get the two new additional disks?
Is this an FC10 or something similar?

A6255A (16 disk enclosure, 4 channels max)

SolutionIt maybe something wrong with your JBOD's Part Number, according to this P/N you have an DS2405 wich holds 15 disks and 2 controllers (Each with two connectors).
If you have this hardware, make sure the both LCC's have the same ID in the rotary switch.
