Hello,
I've just installed patch PHCO_25424 with dep, with automated package created with create_depot_hp-ux_11, and I've got an error message at startup and have to look in /etc/rc.log (included file). I notice some errors (swap start...).
When i try to launch the "last "command, i've got the error message:
# last
Memory fault(coredump)

in your log there is another error.
while configuring all unconfigured software filesets
the system couldn't remove the follow:
Obsolete.NCSNCK,l=/,r=B.11.00,a=HP-UX_B.11.00_32/64,v=HP,fa=HP-UX_B.11.00_32/64
check this file:
/var/adm/sw/needs_remove
you can try to remove it manual and see what happened.

Solution
Hi Jerome:
The error for swap activation is due to the fact that you have added an entry (/dev/vg00/lvol2) for primary swap to '/etc/fstab'. Remove this entry. Primary swap was enabled during the execution of 'S100localmount' and then (as it should have been) during 'S500swap_start start'.
As for the 'last' abort (coredump). Your '/var/adm/wtmp' file is probably corrupt. Create a new empty file to repair the error:
# /dev/null > /var/adm/wtmp
