what kind of HW failure are you getting ?
If you find something from your dmesg, this can help.
But it's best to open an HW call.
If failures are coming from all the disks, chances are that are not the disks but the FC, or even the driver.

If you are running EMC disks, have you adjusted the disk timeout value?

What sort of FC array are you using?
Is it the same disk that fails or different disks on the array?
