I have one server with this error:

It is very difficult to comment on ioscan output. Are you upto date on patches??
What does following command show??
pvdisplay /dev/dsk/cxtxdx
diskinfo /dev/rdsk/cxtxdx

On my server if I run ioscan -fn -H
Could you please run:
/opt/resmon/bin/resdata -R 170786833 -r /storage/events/disks/default/0_7_0_0.10.31.0.0.0.0 -n 170786869 -a
and post the results.

Looks like the fiber card or the switch port on the SAM switch is acting up.
You are probably able to use the disk c8t0d0 because you have an alternate path to that disk.
